The author is the person or organisation responsible for the creation or revision of the web site.
Use angle brackets to contain the web address to eliminate confusion with other punctuation.
There is no comma or full stop between the author and date web site created/revised. Commas are used to separate all other elements. The reference entry finishes with a full-stop.
A web address can be given directly in the text, enclosed in angle brackets. An entry in the reference list is not required in this case. (e.g. The University of Melbourne Library's web site <http://www.library.unimelb.edu.au> describes ...)
